Accepting new GYN patients

Bring your actual photo ID and a physical copy of your current insurance card to the appointment.

Please be advised that without a photo ID to verify your identity, we will NOT be to see you as a patient if you wish to file insurance.

We do have a wireless printer that you can use if you can access it electronically on your phone.

All new patients must complete the “New Patient Forms” and the “2024 consent updates” prior to being seen. These extensive forms are first your first time visit only. At subsequent visits, you will only need to complete the established patient forms yearly update (only 3 pages).

Established patients must complete “Established Patient Yearly Forms Update” once a year to ensure correct information is documented. We have updated our consents so that they will no longer require any annual renewal, please also complete “2024 consent updates” if you have not yet updated consents this year. You will not need to update the “2024 consent updates” at any of your subsequent annual visits.

All forms must be printed and brought with you to the patient appointment; they may not be submitted online ahead of time.

The HIPPA form is for your information only.

If you cannot complete the paperwork ahead of time, please come to the office 30 minutes prior to the appointment time in an effort to have it completed by your appointment time. We strive to run on time with our appointments.

We do have a 15 minute late policy. If you are more than 15 minutes late for your appointment, you will most likely be asked to reschedule to a different time or date unless a cancellation has occurred.
